Compativility / ultra wide 2 y ultra wide 3

Having 21 ID on a RAID 5 with 11 hard drives respectably with SCSI ultra wide 3, intermittently an error comes up, as follow: FAILED RAID REQUIRES TO REBUILD REBOOT THE SERVER.
If you do reboot it, the server start up perfectly, but the error persist. The following actions has been take.
Swuap drive cage, upgrade firm wares, replace the ID failing.

Any suggestions will be appreciated.

what kind of raid controller card are you using, any part number? tell me the part number of the hard drives you are using also. There is some issue about Compaq 4250ES controller and 18.2 GB Fujitsu Ultra3 drives. Need to replace system board with spare number 126982-001 and SCSI backplane with spare number 126994-001.
