DL380 G4 PSU 2 Constant Failure

I have a DL380 G4 which keeps displaying PSU2 has failed. We have replaced PSU2 x5 times, the Motherboard/Power Cables/Power Convertor have all been replaced but we still have the problem.

I'm now looking down the management agent route. Below is what we have installed on this server running Windows 2003 No SP.

This is the only server running this config and the only one with this problem however it was displaying the same symptoms prior to a few of the agents being upgraded.
