DL580 Gen8 K6000 no signal

I run one DL580 Gen8 and have a Nvidia K6000 12GB installed. The BIOS lists the device in slot 9. The card is found and used by Ubuntu 21 both via VNC and in the iLo Console. However the following problems happen:

When I switch in the BIOS to AddOn only, the server boots stops for no reason. Just no response anymore. I have to switch back to both cards, integrated and AddOn.

However, I never get a signal from the Nvidia to the monitor. Just blank. Any similar issues?

Please do perform the below.

1. Reseat the card and cabled

2. Update the frimware and driver of the card.
