DL580 with embedded Smart Array 2 running NT

I'm having a problem in my environment with DL580 (G1) servers (embedded Smart Array 2 controllers) running NT 4.0. I keep a disk image of NT 4.0 (starts at GUI setup) that I load on to any/all of the servers I have of this type. After imaging and completing the setup, I immediately try to install SP6a. Every time, on any server, while the SP is decompressing/installing, one hard drive will drop off the array (raid/5) and start to rebuild. The problem seems to be related to high disk activity, but not a hardware issue, as it happens on several machines. Windows event viewer lists an I/O error on the scsi port device (array controller) when this happens. I've updated the O/S array drivers, array controller and hard disk firmwares but nothing seems to correct this issue. Has anyone else experienced anything like this? Any suggestions?
