Error 45 when running Smart Start

Error 45 when running Smart Start

I get a 45 error when I am trying to run smart start. I have run a system erase many times. I am on the latest firmware for the motherboard, and controller card. Any ideas?

Try using another SmartStart CD , if not using current 4.90 SS CD update .
