Flash Upgrade of array controller fails on DL580

Booting from the latest firware upgrade CD v1.50, trying to flash the embedded array controller from v1.42 to 1.50 fails. The installation log doesn't give any error numbers. I haven't been able to find any support documents on the HP site. Does anyone know why this is happening and how we get the flash upgraded?

what if you try applying Firmware CD booting the server with that Cd? doing it offline

I tried booting from the firmare cd as well and it fails. The error message displayed gives no error codes or any useful information. It just says the firmware upgrade failed. I've tried from a cold boot as well.

This is the embedded controller - I believe the model is 4040.

A side note: there was also a 532 controller installed in a PCI slot and this also failed when attempting to upgradet it's firmware. I have since removed this card for troubleshooting. I thought it was strange this card firmware wouldn't update either.
