HP ProLiant ML150 G6 - Graphics controller failure?

I have HP ProLiant ML150 G6 server with FreeBSD 8.0 installed. It worked fine about 6 years, but suddenly it ceased to display anything. When server was rebooted, there is corrupted splash screen image appeared, then goes to black screen. However, server starts properly, all services works fine and OS is accessible through SSH.

How can I make sure that the video card is defective?

 
What video card models are recommended to replace the integrated video adapter?
