Re: HP Proliant ML570 with Smart Array 6400 controlller problem
Have you connected any new enclosure/ device to any internal / external SCSI connectors of SA card? if yes, change the position of the newly added device. an SA 6400 series card have 2/4 connectors, both external & internal. if you connect devices both on external & internal connectors of same port, this error will occur.

Re: HP Proliant ML570 with Smart Array 6400 controlller problem
The card running at 133 Mhz.
Have you been swaped the cabel to the channel A of the internal connector?
Are you sure you haven't add somethign on the external A channel connector?
Can you do an ofline Rom Upgrade of this controller?
You can alway plug out the additional Cashe and see if the system then boots smoothly.
Sincerly.

Re: HP Proliant ML570 with Smart Array 6400 controlller problem
Thanks for your supports. I thinks the more I describe my problem, the more I have better suports.
I make sure pluged SA 6400 to correct speed PCI bus. I have not been swapped cable to the chanel A of internal connector and added anything on the external A chanel also. My system have been configured correctly 1 years before. Last weeks, 1 of 3 HDD failured and has been removed from the server. During this time, the server can boot from Logical Disk (RAID-5 from 3 HDD). Yesterday, my server could'nt boot from HDD (required boot from CD or A driver, NIC) and an error message I've shown you. Today, a new error message displayed:"1783-Slot 2 Driver Array Controller Failure [Command Failure (cmd=11h, err=020h)]".
p/s: I can do offline ROM upgrade of this controller. I'm Vietnamese so, sorry about my English.

Re: HP Proliant ML570 with Smart Array 6400 controlller problem
no problem with your english, it's not bad.
I'm affraid that your SA card gone bad. the only option is:
1. try to reseat the SA card, or change it's slot.make sure to completely power off your system when doing this.
2. reseat firmly the accelerator option installed on the card, if any.
3. upgrade the firmware of card.
If all this works for you, you are lucky! other wise change the SA card.

Re: HP Proliant ML570 with Smart Array 6400 controlller problem
I had a similar problem once in the past with exactly the same 6400 controller.
I believe your company in down for the moment? Have you thinked about to call HP?
I'm sure they still support 570 even the are out of warenty.
Sincerly.
