What would be the cause?

Re: Installing Windows Server 2012 on an HP Proliant ML30 Gen9 Server - Erro

From the screen shot it looks to me that the error is occuring during the IP-assisted provisioning stage of the OS - in other words the Microsoft's native OS install has NOT started yet. Since this is Gen 9 box I would check the latest and greates IP 2.x release and get the ISO recovery media and flash the IP on your server.

The IP software is Linux based and frequently updated. During the HPE - HP merger separation they were updating IP every month since all the URLs changed. Now that's been solved but older IP software is just useless (assuming you are not running the latest for Gen 9)

Other than older IP I can only assume a hardware failure at this point but looking at the PCI device error I truly think updating IP will solve the issue. (Gen 8 use IP 1.x; Gen 9 use 2.x and Gen 10 use 3.x)
