ProLiant 400 Network card

Our onboard network card failed, this was replaced with a PCI network card. Worked for a few weeks then it also failed.

Does anyone have any ideas as to what could be causing this?

May be you should consult an electrician.
Unwanted electric currents can occur if there are (copper) network connections between different parts or floors of a building.
(May be something from the range of potential equalization and different ground potentials).
Has somebody changed the electric or network cabling in the time before the network cards failed?
