Proliant 6400r cluster

We have a two 6400r in a clustered enviroment. These two servers are attached to one fiber switch, which then attach to TWO fiber arrays. When the ACTIVE server in the cluster is up we find to have errors regarding DISK, and DISK TIME OUTS. We've updated the bios on both servers, the drivers, the firmware on the controller. Also replaced TWO hard drives. At this moment we're still receiving this error message. Any ideas?

What OS is installed?
Where are you seeing these errors?
Include the full error message(s), including any error codes.
What fibre switch is being used? What FC array?
