SCSI Port 1, Drive ID 0
HDD is OK
SCSI Port 1, Drive ID 1
this one has Fl Rd Recv (Fail Read recovery)
which means has some sectors with problems
I think you replaced SCSI Port 1, Drive ID 0?
6i controller firmwre 2.68 (8 Jun 2006)
when the latest is 2.84 (15 Jul 2008)
so you can try this
- First update ADU and ACU to more recent versions
and check if error comes up again
- second update firmware for controller and check again status.
- take out and put back the HDD you inserted to rebuild
- run erase utility on controller - power off server and remove both HDD and run erase utility
- if that does not fix issue recreate RAID

Take complete data backup and system state backup.
Delete the logical drive and recreate the logical drive and reinstall the operating system. and restore the data.
This should solve your problem.

I don't know if there's a official "fix" for rebuilding arrays with one hd with read errors.
If not, my suggestion is that you make a ghost image from Disk 1 to an external hd, then, after swapping the Disk 1, create a new array.
After this, load your previously backed up ghost image to this new array.
Otherwise you have to reinstall all server from scratch :(
