Firstly it's not clear where RIS table mismatch on the Existing or newly added
Assumption - newly added
Pull suspected Drive out
Verify if you have all latest Updates for Array controller and Firmware for all existing Hard Drives

Create only diskette # that needed
If you don't have additional Server
Pull good drives OUT and Install suspected in - run erase Utility or SS Cd 5.5

Update firmware for it after
if it goes trough, say uh if fails say oh
For server if system Partition installed, after erase you loose NVRAM
Uses for F10 access restore when existing Drives installed back

Install new Drive and try one more time

Also what about the array controller? It has the latest firmware and drivers. Will I have to roll back to something older?

Nothing Wrong to Try Updates for Driver, Firmware for Array Controller, ACU, firmware for Hard drives. You can try those steps as first troubleshooting. It much easier to: update; than reinstall. You can leave all three HDD in the system and boot from diskettes for HDD Firmware. If some Updates had done already to the latest, leave them, no downgrade needed. If Update won't help, try to wipe at least third drive and reinstall back. If RIS Copy mismatch on the existing Drive, it's up to you to leave like that or do complete reinstall, however Update still needed if they not that in that case.
