Smart Array 641 reporting "Parity Initialization Failed"
The HP Array Diagnostics Utility is reporting Smart Array 641 card "Parity Initialization Failed".
What does this mean? Could this be caused by a failed Controller Battery?

Possibly, more likely a broken disk. Any read errors on any of the disks seen in the ADU?

The ADU is reporting the following:
Smart Array 641 in slot 6 : Device Error Report
Device Severity Error
--------------- -------- ----------------------------
Logical Drive 1 Critical Parity Initialization Failed
All of the drive indicate that they are OK.
