Depends on the version of O/S you're using.
There are different kernels for single and multi-processor configured systems.
Also it depends upon how much memory you want to address and the version of Windows you're running. Some 32-bit versions have issues with anything over 4GB.

A reinstallation is in many cases not neccessary.
if the first processor allready is a hyper-treading processor then windows included multiprocessor support during installation.
So adding an extra processor makes no difference from the OS pont-of-view.
If it was not, depending on the windows-version you may need a single-to-multipcosessor utility to configure windows to use both processors.
It's allways a good idea to have a recent backup at hand before doing this.
