Re: logical drive state is 04
Post Error - Message: 1786-Drive Array Recovery Needed - Error: 86
If the two are related this means Logical Array State 04 = Drive Array Recovery Needed.
This tells me what I already knew, what I was hoping is this telling me why it's not recovering... Even if the auto-recovery is on and no new read/write error are occurring + all disks report OK.
Can I force the recovery to take place?

SolutionDrive 0 timed-out. Throw a hot spare drive in, configure it and see if it recovers.
Recovery with old controller & disk firmware is hit or miss. Update the controllers, disk drives to the latest firmware revision and update your ACU/ADU to the latest supported versions to avoid these issues in the future.

Re: logical drive state is 04
You could try this to force a rebuild since it is RAID5:
Backup the volume!
Replace Drive ID 4 in physical slot *5*
Physical Drive State: (00=OK, 01=bad or not present, 02=replacement)
01 01 01 01 01 01 01 01 01 01 01 01 01 01 01 01
00 00 00 00 *02* 00 01 01 01 01 01 01 01 01 01 01
01 01 01 01 01 01 01 01 01 01 01 01 01 01 01 01
01 01 01 01 01 01 01 01 01 01 01 01 01 01 01 01
Signature: 0xa00e2264
RIS version: 1501
Logical Drive State: (00=OK, 01=failed, 02=unused)
04 00 00 00 00 00 00 00 00 00 00 00 00 00 00 00
00 00 00 00 00 00 00 00 00 00 00 00 00 00 00 00
After the rebuild completes check the array status with ADU, if there are no more errors being reported, update the controller, drive firmware and ACU/ADU.

Re: logical drive state is 04
The previous disk on that slot started behaving strange and I had it replaced. Logical array is in "ready for recovery" status and not "OK"... If I move any other drive than replacement I loose data.

Re: logical drive state is 04
Not much choice other than than to try to reseat the drives. Could be a flakey slot?
FWIW: I've seen other sites just live with the issue until something else failed and it finally became unusable....
Best of luck.
