orange tape cleaner auto-ejected, why??

We have an HP Ultrium-1 SCSI tape drive whose orange 'Clean' light is flashing.

Upon inserting the orange cleaning tape, it is immediately ejected.

Does this mean the tape drive is beyond repair or that the orange tape cleaner must be replaced??

How many times have the cleaning tape been inserted.

For DLT, it can be used 20 times only, and if you put it in more than 20 times, you will get that behavioer.

I belive its the same for LTO.
