Life Keeper problem on 380 G2 Cluster package (MSA5000)

A customer is experiencing cluster failovers
when getting a event=inqfail on one of the
msa500 raid5s. Lifekeeper prints out several
errors than the system reboots failing over to the other DL380. In the messages there
is a LUN number. I can't find information
on how to interpret the LUN number or
the "ingfail" event. Does nay one have information on these or seen this problem.
