RAID array configuration error code

When I try to upload the Cisco IP Telephony Server OS 2000.4.5 installation and recovery disc onto the Cisco Call Manager, I get the error code 2822. I have a HP Smart Array P400 controller in slot 1. Is this possibly a bad Array controller? It configures the system BIOS and clears the RAID array Controller, then directly after that its says, "The configuration of the array controller failed! This can be caused by unexpected hardware or a hardware failure within the system. Error Code:2822
