Fibre Channel Host Controller Status Change

I have two enclosures with 5 bl20p servers each one. Two weeks ago, I've been receivings alerts "Fibre Channel Host Controller Status Change (16028)" from all servers in one of the enclosures with this details:
Trap Flags=4 (Condition Failed)
location = Slot 0
Host Controller Index = 2
Host Controller Status = other
Host Controller Model = fcmcG4
All servers have the same alert...
ILO doesn't show degraded conditions in the servers. Everything looks OK, serves are OK too.

Symptom: Fibre Channel Host Controller Status Change. This trap signifies that the agent has detected a change in the status of a Fibre Channel Host Controller. The variable cpqFcaHostCntlrStatus indicates the current controller status.

User Action: If the controller status is failed, replace the controller.

All my bladeservers's HBA are fine. All servers sends the same message but ILO shows all OK.
I don't thinks that's hardware fail

Then you could losing fibre connection temporaily and regains it fast?

I don't have any log that shows Fibre Channel Connection Lost...

I don't have any Blade Servers, but just a couple of questions / suggestions.
How many HBA's are actuall involved? Does each Blade have an HBA?
If that's the case and all are reporting a drop at the same time then I'd look beyond the HBA at what they're connected to.
Are they all connected to the same switch?
Perhaps look at configuring snmp on the switch to send traps to HPSIM and discovering that in HPSIM.
It could be a switch or zoning issue.

I had to change the SAN Connection Module. Fixed.
