Increased Frequency of UPS AC Line Fail Messages
I'm starting to see an increase in frequency for UPS events with the following text: "UPS AC Line Fail"
Happens 1 to 4 times per week on some of my servers, but not all. Usually withing 3-5 seconds the UPS recognizes the AC power again with the return of the message "Return from UPS AC Line Fail"
Recommendations for debugging this problem?
Is it possible that the batteries need replaced? Does the power need to be monitored externally, hopefully without any outage?

I'd check the power code into the UPS and the comms cable between the UPS and the server first. Make sure they are nice and tight. Do your 'outage's happen at any particlar period of time? How old are the UPS / batteries?

When you don't bother to identify the UPS model or make clear if this is a single UPS or multiplr UPS's, it's rather difficult to be of much help. In any event, I would tend to take the UPS messages at face value.
