Device not ready

Hi all,
I'm getting this errors after loading qla2xxx module for my FC HBA:

Sep 21 10:53:58 inf17 kernel: sd 0:0:0:1: Device not ready: <6>: Current: sense key: Not Ready
Sep 21 10:53:58 inf17 kernel: Additional sense: Logical unit not ready, initializing command required
Sep 21 10:53:58 inf17 kernel: end_request: I/O error, dev sda, sector 0

It have a disk that is mapped to sdb instead of sda.

Anyone knows what is causing this errors and why the disk is on sdb instead of sda?

The module options are:

options qla2xxx ql2xmaxqdepth=16 qlport_down_retry=30 ql2xloginretrycount=30
