Does anyone know the correct driver to use for Windows 98 ?
The HP part number is 5064-1912

Robert:

You could try the drivers at http://www.adaptec.com/worldwide/support/driversbycat.html?cat=%2fTechnology%2fSCSI+Host+Adapters%2f2910+Family
however, since it appears to be an HP part, I for one would prefer to have an HP-blessed driver. I assume this SCSI host adapter came bundled with some HP computer. Can you tell us what it came bundled in? Thanks.
Make a great day!

No idea where it was used. It was in our server room and no one know how it was used. It may have come out of a Kayak but can't be sure.

The part number originates from a HP NetServer E30.

Robert:

If it's a part out of a NetServer, you probably won't find WIN98 drivers for it on HP's website. You may as well try the link I sent you earlier. Good luck!

The card has a 78xx chipset. The AIC78xx.mpd driver works fine on Windows 98.
