Hello,
Only 3 should be on, not 4...
Take care to put the SCSI ID the same asthe other one by comparing the jumpers on the rear of the tape drives. A different SCSI ID will create a different device file, as in your case.
Check whether the cable is fully inserted in the tape drive. If I/O error still displays, you have to replace your tape drive.
