External USB DAT 72 (DW027A) - tape not loaded

After a couple of unsuccessful backups, the "Clean" LED is permanently ON.
Tapes (both data and cleaning) are not loaded.

L&TT detect the drive; If I try to upgrade firmware (ZU8B) I get an error:

SCSI TUR failure with Sense Condition
Sense Information: SenseKey 0x4, ASC/ASCQ: 0x40/0x4 (Diagnostic Failure on FRU 4 - Flash Error)

Any ideas?

Adding to initial post, when I select the device, after scan, I get this message:

"The diagnostic functions are operating in "restricted mode..."

Health, Test and Utility Icons are disabled.
