HP Storageworks Ultium 460 keeps ejecting the tape

Hello,

We have an HP StorageWorks Ultrium 460 which keeps ejecting the tape.   Model Q1520A, S/N HUL6B01502, Rev D012, P/N - Z1520-00400 

The tapes that we use are:  HP Ultrium data cartridge. 

Also, would anyone happen to know what the fan color lights mean?  Right now we have a solid amber. 

We are looking for any ideas to help us.
