I/O Device Error - Compaq Storageworks DLT VS 40/80 SCSI

We use a Compaq Storageworks DLT VS 40/80 SCSI tape drive to perform nightly backups via Veritas Backup Exec 9.1 on an HP Proliant G3 server running Win2K Server. We have been getting frequent failures, and I am beginning to suspect the tape drive. Drivers & firmware have been updated, Veritas software was uninstalled, then re-installed. Any help would be appreciated. Attached are the event logs from last night's backup:
