I/O Device Error on TL891

I have a TL891 connected to a Modular Data Router which is connected to a fiber channel switch. I am getting increase I/O device errors on my backup jobs. Over half the jobs fail with this error. I am using Veritas Backup Exec 8.6. Does this sound like a physcial problem with the tape library? The library has two drives and the error occurs on both drives.
