You have configured zoning on your switches and the zoning configuration is incorrect.
Check the status of the connections, ensure that all devices connected to the SAN have Link up.

Download and install sg3_utils for your red hat version, and run:
sg_scan -i
sg_map
sginfo -l

I had the same issue at several customers and ended up by adding a script that performs a 'hp_rescan' command during the startup of the server.
Just add the following lines to /etc/rc.d/rc.local:
#detect HP Ultrium drives
/opt/hp/hp_fibreutils/hp_rescan -a
