B4 40 means that the cartridge present sensor didn't transition as expected on a pick or a place. Usually it means that the tape stuck in the slot a bit and when the robot tried to pick (pull) the tape out it didn't come out right away. If it's recovered you can ignore it but I recommend removing the magazines and manually pull/place each tape several times.
We have found some non-HP tapes that aren't shaped quite right and stick a bit but a few pull/place cycles smooths out the sharp edge.
