all lights flashing

We have a customer with a sdlt 110/220 sdlt in there Proliant.
the drive has now been replaced 5 times with the same fault Tape stuck in drive all lights flashing is this to do with the drives being faulty ?????

What firmware revision are your tape drives at? We've been advised by HP, who in turn have been told by Quantum, to upgrade to V45. Unfortunately you have to restart the server afterwards (which wasn't in the instructions supplied) but it does seem to make the tape drives a lot more stable.
