d2d4112 stopped working, INQUIRY command failed
As I was trying to load a virtual tape our d2d4112 suddenly stopped working. The web GUI shows healthy status, but I'm now unable to manipulate tapes using the mtx command:
[root@aus02fstore2 host0]# mtx -f /dev/sg0 load 11
mtx: Request Sense: Long Report=yes
mtx: Request Sense: Valid Residual=no
mtx: Request Sense: Error Code=0 (Unknown?!)
mtx: Request Sense: Sense Key=No Sense
mtx: Request Sense: FileMark=no
mtx: Request Sense: EOM=no
mtx: Request Sense: ILI=no
mtx: Request Sense: Additional Sense Code = 00
mtx: Request Sense: Additional Sense Qualifier = 00
mtx: Request Sense: BPV=no
mtx: Request Sense: Error in CDB=no
mtx: Request Sense: SKSV=no
INQUIRY command Failed
Rebooting the d2d did not help. I also tried rescanning the HBA bus but this did not help either. Has anyone else experienced something like this?

What does the web GUI say in regard to connectivity?
Obviously since you can still talk to the appliance via the web GUI IP connection it hasn't stopped working - more likely something between the appliance's FC connection and your server has an issue.
It could be a issue with the D2D FC HBA I guess but the GUI will tell you the status of the FC connection from the D2D point of view.

When the server has finished all its scheduled jobs for the day I will reboot it and see if that helps.
