mkltfs fails with "Format operation failed (-1)" Legitimate request
I have a HPE Ultrium 8-SCSI drive with a HP 1x8 G2 AUTOLDR loader. I'm running HP StoreOpen Software version 3.4.2-9 on CentOS 7. For some reason, I am no longer able to format my LTO-8 type M tapes using the mkltfs command:

Here is a link to a github issue I raised for this at the ltfs github repo. It got marked as wontfix because I'm using HP software and not vanilla LTFS. If anyone could please check this issue and offer advice, it would be greatly appreciated. Thanks!

The most interestest parts in that error message are Sense Key 04, ASC/ASCQ 03/02
That error decodes as Hardware Error, EXCESSIVE WRITE ERRORS
Have you tried multiple tapes? If multiple tapes are failing with the same error code then maybe the write head was damaged.
Are you able to write to tapes that have already been formatted?

Have you tried multiple tapes? If multiple tapes are failing with the same error code then maybe the write head was damaged.
I have tried 3 or 4 different tapes. I cannot format any of them. The other tapes that worked are from the same batch of tapes with barcodes from the same barcode sheet.
Are you able to write to tapes that have already been formatted?
I haven't tried this yet. Most of my tapes are completely full. I'll see if I can find a tape that's not completely full.

Another thing I noticed is that in the web UI I see "Media Attention" in the System Status area. I have a brand new unformatted LTO-7 type M tape in the drive right now. If I click on the "Media Status" link it says "Media Error" in the Drive Inventory - Source section. Is this just because the media is unformatted? The other tapes that I've already written to do not give a "Media Error" status.

Yes, LTO-8 drives can read/write LTO-7 type M tapes. I have already successfully written and read back about a dozen of type M tapes in the past.

I have the ltt tools installed. I'm not sure which test is the read/write test you're referring to. I ran the "Backup Performance Test" but it seemed to finish nearly instantaneously. I got this for the results:
0.429578 MB read (136 files) in 0.010783 seconds with rate 39.838433 MB/sec
Backup Performance Test successfully read files.
I ran the restore performance test:
1020 KB written (8 files) in 0 seconds at 230821 KB/sec
Restore Performance Test successfully created files.
These tests were both done with an unformatted disk in the drive, so I'm not sure if the results are even valid.

I regret to report that I asked a person more familiar with the low level error codes than I am to take a look at this and they reported that there is a low level drive error code in the additional information for the error I pulled out earlier indicating that the failure writing was due to an issue with a write head open circuit.
It looks like there is a write head failure in the tape drive.

Thanks for your help Curtis. I can't seem to find either the "Drive Performance Test" or the "Drive Assessment" test in the ltt tool. I'm using ltt 4.28. Do I need a newer version of the ltt tools to find these tests? It looks like there is a section in the ltt tools to enter a password. I tried entering the numeric access password for the loader, but it didn't work. The only tests I've been able to find are the backup and restore tests. The menu system is somewhat hard to navigate. Can you tell me how to find these tests?
