When the drive cannot calibrate the tape, it cannot read the media ID, so I can't tell if this all happened on the same tape.
Is this a DLT IV tape that was written in a non-DLT1/VS80 drive? If so, it cannot be used without degaussing.
Have you tried multiple tapes? Have you cleaned the drive?
I would recommend running the LTT drive assessment test.

it was the same new tape, formated with this dlt1 drive.
can you tell me how to degaus a tape?
i have problems to degaus old dlt4000 tapes :(

Thanks for clarifying. As I said, it's possible that there may have some debris on the head (or tape) that cleared itself up.
