This all started with an OS X user having problems access a macintosh accessable share on a windows 2000 box. He would lose the connection regularly. The event log shows 2 second drops across all blades. Here is an example:
I 03/19/03 08:29:20 ports: port A5 is now off-line
I 03/19/03 08:29:22 ports: port A5 is now on-line
I 03/19/03 08:39:12 ports: port B1 is now off-line
I 03/19/03 08:39:22 ports: port B1 is now on-line
This is happening to all ports on a 4000M it doesn't seem to matter if they have a windows box or a Macintosh hooked up on the other side.
I did the following:
1. Upgraded the switch to 9.16
2. I checked the wiring from the workstation to the switch. All cables passed and were within cable limits.
3. I add a netgear switch between OS X box and the switch.
4. Verified spanning tree is off.
None of these things fixed the problem.
Next I read Apple support article 107475 that detailed how to lock down the Ethernet to 100 half-duplex. This currently is fixing the problem. I know this is a long-winded sorry.
Questions
1. Is this a known problem?
2. Should I throttle down all ports and workstations to be just 100 half-duplex?
3. Should I try anything else?

Hi.
If the NICs are set to a certain speed/duplex, you should set the switch ports to the same speed/duplex. So, try setting both the NICs and the switch ports to 100FDx.

and would like to say that it didn't help me a bit. I am still experiencing the same problem as described above on all my switches with:
HP J4121A ProCurve Switch 4000M
Firmware revision C.09.22
Event log:
I 10/25/05 19:50:31 ports: port I3 is now on-line
I 10/25/05 19:52:24 ports: port I4 is now off-line
I 10/25/05 19:52:27 ports: port I4 is now on-line
I 10/25/05 20:01:15 ports: port A5 is now off-line
I 10/25/05 20:01:23 ports: port A5 is now on-line
I 10/25/05 20:01:31 ports: port A5 is now off-line
I 10/25/05 20:01:33 ports: port A5 is now on-line
I 10/25/05 20:01:54 ports: port A5 is now off-line
I 10/25/05 20:01:56 ports: port A5 is now on-line
I 10/25/05 20:04:37 ports: port B2 is now off-line
I 10/25/05 20:04:46 ports: port B2 is now on-line
I 10/25/05 20:04:54 ports: port B2 is now off-line
I 10/25/05 20:04:56 ports: port B2 is now on-line
I 10/25/05 20:05:18 ports: port B2 is now off-line
Uptime on devices connected to these ports didn't changed, cables are perfect.
