MEM_FAIL_NOTIFY

i have received following snmp trap from my switch;

2005-09-18 11:20:34 Syslog.Debug [1.3.6.1.4.1.11.2.3.7.11] [Event received from X.Y.Z.K: W 09/18/05 11:16:51 chassis:
Slot D Software exception in ISR at intr.c:347 FATAL SCHAN ERROR S=6 D=5 OPC=20(MEM_FAIL_NOTIFY) ECODE=2

could you help me please, what does it mean ?

What model switch and software version is this from?

Here is a release note for the 4100, where this was fixed in G.07.65:

Oops...Les, thanks for pointing out the attachment. Sounds like upgrading to a newer software will fix this problem.
