Advfs domain panic: I/O erro 5

Sir i had Alpha Server DS20 with Tru 64 unix, it had 2 hard disk having four logical partition like /u01, /u02 & /u03. recently i am getting the problem that /u02 & /u03 become inaccessible aburptly and restore again if i start the server again (hard boot).
message is coming "Advfs domain panic has occured due to either metadat write error or an internal inconsistancy. This domain is being rendered inaccessible.
when i run the verify command, following msg comes: verify: can't get set info for domain
verify: error = I/O error
main: unable to get info for domain
error :5, I/O error

please exchange the disk. It has gone bad.

I/O error means that something couldn't be written or read to the disk. The reason might be several things, but the most common problem would be the disk.
Replace and monitor

if using search within this forum you will get the right answer. First step is always to check the hardware...
