Alpha 1200 Digital Unix V4.0F- Do not print Banner

I have an Alpha 1200 Digital running Unix V4.0F. The print server card failed on a HP LaserJet 2100 (JetDirect 615). Hp replaces with JetDirect 620. Now when a user prints a banner is printed also every second page:

User:
Host:
Class:
Job:

There is a record in /etc hosts for the printer as follows:
IP Address.....L5....l5.......#Comment

How can I supress this?

Update: printcap holds the following:

lp12|12|L5|GI|l5:\
:lf=/usr/adm/lp12err:\

:rp=:\
:sd=/usr/spool/lpd12:

Before my time, but have you tried using
"sb=true" to Suppress the Banner?

"man printcap"

Thanks for the reply. I found the solution. There was a setting on the JetDirect print card 'LPD Banner Page'. By default this was enabled.
