From ioscan, I get:
HP Tachyon XL2 Fibre Channel Mass Storage Adapter
From fcmsutil I get:
@(#) PATCH_11.11: libtd.a : Dec 12 2003, 14:34:15, PHSS_28509
But in EVA docs I see names like:
A6795, etc? How do I match "Tachyon XL2" to a "AXXXX"?
Also, in EVA docs I see driver names like:
"11.11.09", etc. that don't look like the fcmsutil output: How do I find what driver I am using?

Well the A6795 is a Tachyon XL2 2GB fiber channel HBA. I guess you could look it up on PartSurfer to verify.
To find the driver level run:
swlist -l product | grep Tach
You should ge a line like
FC-TACHYON-TL B.11.11.09 PCI/HSC FibreChannel;Supptd HW=A6684A,A6685A,A5158A,A6795A
The B.11.11.09 indicates the driver level. You'll probably also get any Tachyon patches as well like the PHSS_28509 you got from the fcmsutil command.

You can get a LAN card part number from an ioscan...
In general the cards are:
On K's the cards are:
A6685A: Tachyon
A3404A: Old FC-AL.
On all the other new servers, the cards are PCI type:
A5158A: Tachyon or Tachlite. 1 GBit/sec
A6945A: 2 GBit/sec
A6795A: 2 GBit/sec (A-180)
You can shine a flashlight on the back of the server chassis and read the number off the card...
