If its not listed in ioscan then you have a physical problem with the board not being picked up in the i/o backplane.
If you installed this hba yourself then reread the installation procedure and try reseating the board.
Many times a board can only go into a certain slot, one that is capable of supporting it. Usually its a slot with a higher speed.
You may also have an incompatible board with your server.
a) what is the server model and o/s version?
b) what is the product number of the HBA?
You'll have to refer to the packaging or visually examine the unit yourself to find it. It will be something like, A5158A.

If not listed in ioscan the HBA or the slot maybe dead. In this case I would expect more indicators, even a crash.
All the fcmsutil and similar tools will not help, if the HBA is dead.
If this is a new installation, I would try to re-seat the card.
If the card is working but not recognized by the OS, at minimum the firmware is aware of it.
Example - a "display fru":
...
FRU Entry # 5 :FRU NAME: Mezzanine 1 ID:08
CHASSIS INFO:
BOARD INFO:
Mfg Date/Time : 5260320
Manufacturer : QLogic
Product Name : QLogic QMH2462 4Gb FC HBA for HP c-Class BladeSystem

A NIC shows activity (LEDs are blinking), the slot shows populated and powered in "olrad", but ioscan shows the slot as empty.
Re-seating the card was the solution!

To add with the above, If you have a bad fc card , failing or failed, you will see EMS alert , check logs at /var/opt/resmon/log/event.log for further.
- Then check with fcmsutil for status.
- A card may not failed completely may be still working for few days before it fails completely. So a proactive measure would be wise.

First, you need to start a new thread and close this one. Its an organizational and search engine thing and lack of confusion thing.
Second, need to know you architecture. Disk array, SAN, load balancer in particular.
In most data centers the network group takes care of the SAN connectivity and zoning, whatever, if you are presenting a disk from a disk array through the SAN to a server, then there are several places along the way for you to stop and check.
For example, if you have the EMC power path load balancer, you run powermt display and all the luns from the disk array for your server are listed. Now you look at the device path and line them up in ioscan, which should also be listing out everything.
Question: Are you not seeing a lun in 'ioscan'?
