save_config:error-unknown disk type for not SCSI or HPFL
save_config:error-cannot determine root disk

I am using /dev/rmt/0m
when I do a bdf / is lvol3
I am on 11.00

Can you post the output of:
vgdisplay -v
and
strings /etc/lvmtab

I susspect you have a corrupted lvmtab file and need to resore it with vgscan after you save the old /etc/lvmtab file to lvmtab.sav or something like that.
If it has wheels or a skirt, you can't afford it.

sorry the correct vgscan would have the -a switch:

vgscan -a

strings /etc/lvmtab

If all as well there, it may merely be a bug.

I found removing vxvm fixed these errors.
