Inquiry failed on SAS/SATA

Hi, we have a problem with some messages apearing in system log. The error in syslog.log is:

 

SAS driver at 0/4/1/0 (/dev/sasd0) : Inquiry failed on SAS/SATA device with address 0x5000c500013f49d1

 

I attached the last output of event.log in this post and attached to the output of sasmgr command. Please, can anyone help me to understand whats happening?

There are visible dsks in slots 1, 2, 3, 5 and 6 - do you have a dead disk in slot 4?

Is the error reoccuring in the event.log? 

Check for any corresponding errors in syslog.log
