Meanwhile you did the vgcfgrestore? Maybe the "other" disk added to the VG is now missing. How does the vgdisplay now looks like?

i'm about to be a dreadful person. when i researching support issues here, i hate it when the person with the issue says "we're going to solve <the problem> in an utterly different manner".  (sorry world)

it turns out there is an unused 146Gb drive in the system. i'm going to create a new vg ('vgnew') and two lvols and two new filesystems/directories. i'll copy everything off of vg01 onto vgnew (glossing over the details here). once we're satisfied that vgnew is sane, i'll get rid of vg01 and now that the problematic 146Gb drive is unused, i'll use it to mirror the lvols in vgnew...with no PVG(!)

(it's like hgtv (or maybe hptv where i don't like the paint colors in my house so i want to move
