for p2vassist command Do I need to install hpvm 4.1 on the physical server as well. ????

You should use 11.31 and HPVM 4.1 on the host anyway - it is much better than anything else.

It only migrates the Applications...

I have a Physical server A to be migrated to HPVM A1.

After the migration Server A will be decomitioned... So all the filesystems and Applications etc.. present in Server A should go to HPVMA1

The p2vassist looks like not fully finished, if you read the page.

However, consider to use ignite to transfer the OS.

if the data resides in the SAN, then you only need to take an ignite backup and restore to the VM, modify zoning to present the luns to the VM and import the disks

yes, the other VGs/DGs are coming when you are importing they. Basically, the ignite backup should only be used for the system itself, for other data another backup system should be used.
