Shalom,

Usually this error means there was a problem writing the files to tape. The backup should be re-run, probably with new media.

1) Usually tape write error.
2) Bad tape, tape not in the tape drive.
3) Yes probably, could also be a lack of space in /var to write the file list.
4) You already have by posting. There is a log file visible with smh/sam
5) Can't control bad tape media or your operations people not putting the tape in the drive can you?
