I have an RP7410 and I get the following message, from time-to-time:
SCSI: Unhandled phase mismatch (MESG_IN_PHASE) -- lbolt: 52692, dev: cb022..
The message details are very extensive
Any ideas, anyone ?

It usually means that a driver is trying to negotiate a wide data transfer on a device that doesn't support them. Without a bit more of the error message it's difficult to tell more.

What does ioscan give you?
device major cb(hex) = 203 dec = (normally) sctl/ctl, which doesn't tell you much. Minor number 022002 looks like a tape drive. Have you recently connected a SCSI device to the server?
