You're not giving us much to go on there are you!
What type of disk array? EMC? Clariion? VNX? Something else?
What release of the OS? 11.11? 11.23? 11.31?
How are you using the disks? In an LVM volume group? VxVM disk group?, ASM? Raw? What configuration do you have?

try to check if the power path version is compatible with your OS. we had something like this with powerpath 5.2. when we had it downgraded to 5.1, it did not show up anymore. not sure though if this is related to the issue you are encountering.
