Did you check other log files - syslogs, binary logs, /var/adm/messages etc?
From this output, it looks like a Memory channel problem.

Hi,
did Oracle stop or the machine?
Use decevent or webes for better error details.
Have you looked for the alertlog?

Above sentence in the output says it all:
It was a panic and it happened because both memory channels had a problem. Very bad because you counted on them for redundancy.
Do you have memory channel HUBs ?
Any optical fiber MC extensions ?
And as Michael said, uerf is obsolete, it supports only pre-AS4100 CPU's and devices.
For more recent configs, install WEBES from layered product CD and use command:
wesea n trans out /tmp/errlog.txt full
