ignite error
I am getting following error
NOTE: make_recovery(103): The make_recovery command will be replaced by
make_tape_recovery. Both commands are supported in this
release (Ignite-UX Revision 3.2). Please read the man page
make_tape_recovery(1M) for usage of the new command. In a
future release, make_recovery will be replaced by a script
that calls make_tape_recovery.
Option -A specified. Entire Core Volume Group/disk will be backed up.
make_recovery(284): Writing to the tape failed. Please check the existence of the tape, or the tape is write protected, or the tape is bad.
make_recovery(210): Encountered a fatal error.
make_recovery(212): Please fix errors and re-try.

Which commands did you issue ?
it sounds like command incompatibility ..
And which ignite version AND UX version do you have: check them following commands:
uname -a
swlist -l product |grep -i ignite

Please check if the tape is accessible before issuing the command.
Also let us know the ignite command used for backing up.
Confirm the tape device name.
# ioscan -fnC tape
# mt -f /dev/rmt/tape_device_name stat ## What is o/p?
You can use:
# make_tape_recovery -x inc_entire=vg00 -a /dev/tape_device_name

As per your logs,
you did not put the tape on DDS/DAT drive or
may the tape drive faulty/heads need to be cleaned/serviced.--->try to put cleaning tape and check on tape drive LED status.after putting tape LED for tape--> should green,if orage blinking/glowing then tape drive need to be cleaned either by cleaning tape/manual.
(If you suspect tape drive,contact service Engg)
above steps done then may the tape/media faulty
