The disk at hardware path 1/0/14/1/0/4/0.0x50014380025dc7d9 has gone offline.
Have you checked that disk? Do an 'ioscan -fnC disk' and see what the status is.

we use the ioscan -fnC disk but we are not able to find any disk in that particular
hardware path.
But hp give some solution, but we are not that.
Cell Contents: 4207001b; Indicated Virtual Disk is transitioning the ownership to the other HSV300 controller.
CAC: 00 Port: Position: AL-PA: Sk/Asc/Ascq:
Corrective Action: No action necessary.
Advisory: VDisk was moved to the other Controller since most Reads came in from that. Please read the text in the bottom for Diskname.
Firmware: 09522000
Baselevel: CR153F HSV300
Brief Description: 4207001b; Indicated Virtual Disk is transitioning the ownership to the other HSV300 controller.
Reason: Implicit transition;

There is no any issues in the EVA, Regarding the cluster went down issue in your windows host, you need to have a multipathing installed in your windows host so that the ownership change of EVA Luns doesn├в t affect the production.
You please check with your windows team regarding the multipathing installed. And also check for any other issues which caused the cluster to go down.
